R-Lipoic Acid vs Alpha-Lipoic Acid: The Letter That Halves Your Dose
Alpha-lipoic acid is sold almost entirely on a milligram number, which is unfortunate, because milligrams are close to the least informative thing on an ALA label. Two products at the same dose can deliver quite different amounts of the biologically active material, and the difference comes down to a single letter most bottles never print.
What ALA is
Alpha-lipoic acid is a compound your body makes in small amounts and uses in energy metabolism, where it works as a cofactor for enzymes involved in converting nutrients into usable energy. It's also studied as an antioxidant, and it has one genuinely unusual property: it is both water-soluble and fat-soluble, which means it can operate in parts of the cell that most antioxidants can't reach at the same time.
That dual solubility is the legitimate reason ALA attracted research interest, and it's the part of the story that survives scrutiny.
The R and the S
Here is the detail that decides everything else.
Alpha-lipoic acid exists as two mirror-image forms — R-lipoic acid and S-lipoic acid. They're structurally identical in composition and opposite in three-dimensional arrangement, the way your left and right hands are.
Only R-lipoic acid occurs in nature. It is the form your body produces and the form its enzymes are built around. S-lipoic acid does not occur biologically; it exists only as a by-product of chemical synthesis.
And this is where the labels get interesting. Most inexpensive ALA on the market is what's called racemic — an approximately 50:50 mixture of R and S, because standard chemical synthesis produces both and separating them costs money. So a bottle advertising 600 mg of alpha-lipoic acid is, in most cases, delivering roughly 300 mg of the naturally occurring form and roughly 300 mg of a synthetic mirror image.
The label isn't lying. It contains 600 mg of alpha-lipoic acid. It simply isn't telling you that half of it is a form your biology never evolved to use.
Does the S form do anything?
Fairly assessed: the S form is not understood to be harmful, and it isn't inert filler in a strict sense — it has some antioxidant activity. What it lacks is the specific biological role of the R form, since the enzymes that use lipoic acid are shaped for R.
There's also some research suggesting the S form may compete with R for uptake, which would make it a mild liability rather than merely neutral. That evidence isn't strong enough to state as fact, and it's worth knowing as a plausible consideration rather than a settled one.
The reasonable position: racemic ALA is not a bad product, but you are paying for a milligram figure of which roughly half is doing the work you had in mind.
Why R-only costs more
Producing R-lipoic acid alone requires either a stereoselective synthesis or a separation step, both of which are more expensive than making the mixture. That cost is the entire explanation for the price gap, and it's why racemic dominates the shelf.
R-lipoic acid is also less stable than the racemic mixture — it can polymerise with heat and moisture — which is why the better R-only products use a stabilised sodium salt form and why storage matters more than it does for most supplements.
The practical comparison, then, isn't price against price. It's price per milligram of R. A 600 mg racemic product and a 300 mg R-only product deliver comparable amounts of the active form, and once you frame it that way the premium usually looks smaller than the shelf suggests.
Timing and absorption
ALA absorbs better on an empty stomach — food, particularly a substantial meal, measurably reduces uptake. Taking it thirty minutes before a meal or a couple of hours after is the usual approach.
It also clears quickly, with a short half-life, which is why divided doses are common rather than one large one. And it's worth knowing that ALA is a chelator — it binds metals — so separating it from mineral supplements by a couple of hours is sensible.
One clear caution: ALA interacts with glucose metabolism, and anyone taking medication that affects blood sugar should treat it as a conversation with their doctor rather than an independent purchase. That's not a formality; it's the specific case where an interaction is plausible.
Reading the label
Three checks, and the first one settles most of it.
Does the label specify R-lipoic acid, or does it just say alpha-lipoic acid? If it doesn't specify, assume racemic and mentally halve the number. Is the amount disclosed on its own line rather than inside an antioxidant blend? And if it's an R-only product, does it use a stabilised form — sodium R-lipoate — given the stability issue?
The first check is the one that separates buyers who understand this ingredient from buyers looking at milligrams. A brand that has paid for R-only will say so on the front of the bottle, because it's the reason to choose them.
