Iron: The Marker Most People Guess At

Iron is the nutrient people are most likely to guess at, and the one where guessing carries the most cost. With most minerals, taking a bit more than you need is mainly a waste of money. Iron is different. The risk runs in both directions — too little is a genuine problem, and too much is its own genuine problem — which makes iron the one supplement where the honest first step isn't buying anything at all. It's finding out where you actually stand.

Why iron is the two-sided nutrient

Iron's central job is to sit at the core of haemoglobin, the protein in red blood cells that carries oxygen from your lungs to every tissue in your body. That is not a peripheral function. When iron stores run low, the body's capacity to move oxygen around is affected, and the everyday experience of that tends to be a flat, heavy tiredness that sleep doesn't fully resolve.

But your body has no efficient route for excreting excess iron. Once it's absorbed, it largely stays, accumulating in tissue over time. That one piece of physiology is why iron sits in a different category from, say, vitamin C, where surplus is simply passed. With iron, surplus is stored — and stored iron that keeps building is not benign.

So iron is bracketed on both sides. Not enough, and oxygen transport suffers. Too much, and you're accumulating something the body can't easily shed. The useful range sits between those, and the only way to know where you are within it is to measure.

Test, don't guess

This is the part worth taking seriously, because it's genuinely different advice from what we'd give about most nutrients. Tiredness is the most commonly cited reason people reach for an iron supplement, and tiredness has a very long list of possible explanations — sleep, stress, training load, diet, and plenty of things that have nothing to do with iron at all. Buying iron because you feel tired is reasoning backwards from a symptom that points in a dozen directions.

A blood panel resolves it. Ferritin is the marker usually used to assess stored iron, and it's typically read alongside other measures for context. It's a routine, inexpensive test, and your doctor can order it and interpret it properly — which matters, because the numbers need reading in the context of your wider health rather than against a chart you found online.

Getting tested does two things. If your stores are genuinely low, you now know that, and you can supplement with a clear purpose and re-test later to see whether it's working. If they're fine, you've saved yourself from adding a mineral you don't need to a body that can't easily get rid of it. Either outcome is worth the price of the test.

Who is most likely to actually be low

Some groups are more likely than others to come up short, and knowing whether you're in one is a reasonable prompt to get tested rather than a reason to start supplementing blind.

Menstruating women lose iron regularly and are among the most commonly affected. Pregnancy raises requirements substantially, which is why iron is a standard part of prenatal care and a conversation to have with a healthcare provider rather than to manage alone. People eating vegetarian or vegan diets are relying on non-haem iron from plants, which is absorbed less readily than the haem iron in meat, so intake has to be higher to land in the same place. Endurance athletes lose iron through sweat and the mechanical demands of heavy training. And anyone who donates blood frequently is, by definition, exporting iron on a schedule.

Being in one of these groups doesn't mean you're low. It means the prior probability is high enough that a test is a sensible use of ten minutes.

Why iron has such a bad reputation — and why it doesn't have to

Ask anyone who has taken a conventional iron supplement and you'll usually hear the same complaints: nausea, constipation, a stomach that objects. That reputation is real, and it's largely a story about form.

The traditional forms are simple iron salts — ferrous sulfate being the classic — and they're cheap, which is why they've dominated for decades. The problem is that a substantial share of the iron in these forms isn't absorbed, and unabsorbed iron sitting in the digestive tract is precisely what causes the discomfort people associate with the whole category. The side effects aren't a mysterious property of iron itself; they're a consequence of delivering it in a form that leaves a lot of it behind.

Chelated iron works differently. In a bisglycinate chelate such as Ferrochel®, the iron is bound to two molecules of the amino acid glycine, so it's absorbed through amino-acid pathways rather than depending on the harsher route the salts take. In practice that means better uptake and a much gentler experience — which matters more than it sounds, because an iron supplement only helps if you can tolerate taking it consistently. A cheap bottle you abandon after a fortnight has done nothing for your ferritin.

Small habits that change how much you absorb

If you are supplementing, a few practical details make a real difference.

Vitamin C improves the absorption of non-haem iron, so taking iron alongside a source of vitamin C is a simple, free way to get more from the same dose. Conversely, tea and coffee contain compounds that bind iron and reduce uptake, and calcium competes with iron for absorption — so iron taken with a milky coffee is working against itself. Separating iron from those by an hour or two is usually enough.

And re-test rather than assuming. Iron status changes slowly, and the point of supplementing is to move a number, not to take a capsule indefinitely out of habit. A follow-up test tells you whether it worked and when to stop.

One safety point that deserves its own paragraph

Accidental iron ingestion is one of the leading causes of poisoning in young children, and iron supplements are frequently attractive to them. Keep iron in a closed container, out of sight and out of reach, and treat it with more caution than the rest of your supplement shelf. This is not a boilerplate warning; it is the single most important sentence in this article for anyone with small children at home.
