Myo- vs D-Chiro Inositol: Why the 40:1 Ratio Matters
Inositol is one of the few supplements where the number printed on the label is less important than a ratio most shoppers have never heard of. Two products can both say "inositol," both list the same total amount, and behave quite differently — because inositol is not one compound, and the proportion between its two main forms is the thing doing the work.
What inositol is
Inositol is a sugar alcohol that your body both makes and obtains from food, found in fruits, beans, grains and nuts. It isn't a vitamin in the strict sense, since you can synthesise it, but it behaves like a nutrient in that intake influences status.
Its main role is structural and communicative: inositol is a building block for molecules embedded in cell membranes that act as second messengers. When a hormone arrives at a cell, it usually doesn't enter — it docks at the surface and triggers a relay that carries the signal inward. Inositol-derived molecules are a major part of that relay. So inositol sits inside the machinery by which cells hear and respond to hormonal signals, which is why it comes up in conversations about metabolic and reproductive health.
The two forms
Inositol exists in several stereoisomers — same atoms, different spatial arrangement — but only two matter commercially.
Myo-inositol is by far the most abundant form in the body and in food. It is the default, the one your tissues hold most of, and the one most research has used.
D-chiro-inositol is produced from myo-inositol by an enzyme that converts one to the other. It is present in much smaller quantities, and the conversion rate differs between tissues — which is the whole reason the ratio matters rather than the total.
These are not interchangeable, and taking more of one does not substitute for the other.
Where 40:1 comes from
The number you'll see on well-formulated inositol products is 40:1 — forty parts myo-inositol to one part D-chiro-inositol.
That ratio isn't a marketing invention or a formulator's preference. It reflects the proportion in which the two forms are found in healthy human plasma. The reasoning behind 40:1 products is straightforward: if the body maintains the two at roughly that proportion, supplementing at that proportion respects the physiology rather than overriding it.
The corollary matters more than the ratio itself. Because D-chiro-inositol is the minor partner, loading up on it is not a case of more being better. Research interest has specifically included the possibility that skewing the balance too far toward D-chiro is unhelpful — a reminder that with signalling molecules, proportion can matter as much as quantity. A product that leads with a large D-chiro number is not offering you a stronger version of the same thing.
Why plain "inositol" on a label is ambiguous
Here is the practical problem. A label reading simply "inositol 2,000 mg" is almost certainly myo-inositol, since that is the cheap and abundant form — but it hasn't told you, and it certainly hasn't told you whether any D-chiro is present or in what proportion.
Sometimes that's fine. Myo-inositol alone is a legitimate product and much of the research used it that way. But if you came looking for a 40:1 formulation because you read about the ratio, a bottle that doesn't name its forms is not that product, and no amount of milligrams makes up the difference.

Inositol is also slow. Like most things that work through supporting normal signalling rather than producing an acute effect, it is evaluated over months, not days. Anyone expecting to feel something in week one has the wrong expectation, and that mismatch is the most common reason people abandon it early.

If it's a combination product, does it state the ratio, and is that ratio 40:1 or something the manufacturer should explain? How many capsules or scoops make up the stated serving? And is there independent third-party testing with results you can open, tied to the batch you're buying?
